Questionnaires

A Questionnaire can be used as a mechanism to obtain information from consumers, patients or healthcare professionals.

The Clinical Data Definition template enables an unlimited number of individual Clinical Data Definitions to be brought together into an assessment or questionnaire using the concept of Relationships. In this way Smoking Assessment, Exercise Assessment, Care Assessment and others can be collated together into a single questionniare for risk assessment.

There is no limit to the number of times that an individual Clinical Data Definition can be re-used in different questionnares. For example, Smoking Assessment could be used separately or as part of Breast Cancer Intake Questionnaire, Cardiac Assessment Questionnaire or many other questionnaires. However, Smoking Assessment only needs to be defined once as an Clinical Data Definition itself.
