[edit] Endorsement
We need a way for someone or some organization to digitally "sign" a specific version of an article. While this does not "lock" the article (Clinical Data Definition or whatever) it provides a way to indicate the entries that have been approved by a given authority, institution, etc., or simply to verify that a given Clinical Data Definition has been approved for use by a particular system, authority or institution.
Signing requires a digital certificate issued by some authority like Verisign. The signature must be applied to the content of the article so that a viewer can be certain that the article has not been tampered with in any way.
[edit] Provider Endorsement
Endorsements can be provided by anyone. For example, an individual provider may endorse individual items for use in that one practice. Alternatively, an institution may provide an endorsement. A system may also need to indicate that it supports a given version of a given Clinical Data Definition. This would be necessary to indicate to a user that the system they use supports a Clinical Data Definition that the user wants to use.
[edit] Organization Endorsement
This could be redundant with provider endorsement, but in any case, there may be a hierarchy of endorsements that make something available. Local, state, and federal agencies might impose some authority over the use of a particular Clinical Data Definition. For example, the U.S. Food and Drug Administration may approve specific drugs for specific treatments. A medical society or department may approve certain Clinical Data Definitions. A simple solution is to leave the decision-making to the individual account administrators. Thus, someone will decide to include or exclude a particular Clinical Data Definition based on their own decision-making process which may, in part, include direction provided by various organizations.

For a particular archetype to actually come into use by a user in a system, both the user and system would have to endorse the archetype. The system to indicate that it supports the archetype and the provider indicating that it wants to use the archetype.
